A birth chart (also called a natal chart) shows the calculated positions of the Sun, Moon and planets at a moment of birth. Astrological traditions interpret this configuration symbolically; it's neither a personality measurement nor a prediction.
The Moon changes sign roughly every two and a half days; on a change day, the time decides your Moon sign. The Ascendant moves on even faster, about every two hours, and additionally needs the birthplace.
The positions are calculated astronomically. Without a birth time we use 12:00 noon in the chosen time zone as an explicitly marked reference value. The Moon especially can differ on a change day; the Ascendant and Midheaven aren't shown without a time and place.
